Fitness Class Participation Waiver
In consideration of being allowed to participate in Fitness Classes provided by “iLivFit” or any of it’s appointed instructors, The participant hereby agree to the following terms and conditions:
- ASSUMPTION OF RISK: I acknowledge and understand that engaging in fitness classes and activities involves inherent risks of physical injury and property damage. I voluntarily assume all such risks and understand that participation may lead to physical exertion, discomfort, and potential injury.
- FITNESS LEVEL: I affirm that I am in good general health and have no medical conditions that would prevent me from safely participating in fitness classes. I understand the importance of consulting with a healthcare provider before beginning any exercise program, and I have done so if necessary.
- INSTRUCTION AND SUPERVISION: I understand that the fitness classes are led by trained instructors who provide guidance and supervision during the classes. I agree to follow all instructions and safety guidelines provided by the instructors.
- RELEASE AND WAIVER: In consideration of being allowed to participate in fitness classes, I, for myself, my heirs, personal representatives, and assigns, do hereby release, waive, discharge, and covenant not to sue iLivFit, its owners, employees, and contractors from any and all liability for any and all claims, demands, losses, or damages on account of injury, including but not limited to personal injury, property damage, or death, arising out of my participation in any fitness classes.
- PHOTO/VIDEO RELEASE: I understand that iLivFit may take photographs or videos during fitness classes and related events. I grant iLivFit the right to use such photographs and videos for promotional and marketing purposes without compensation to me.
- EMERGENCY MEDICAL CARE: In the event of an injury or illness during a fitness class, I authorize iLivFit to provide or seek medical treatment as they see fit, and I agree to assume any and all costs associated with such treatment.
- RULES AND REGULATIONS: I agree to abide by all the rules and regulations of iLivFit and to conduct myself in a safe and respectful manner while participating in fitness classes.
- KNOWLEDGE OF WAIVER: I have read and understand this waiver and release of liability and voluntarily acknowledge that no oral representations, statements, or inducements apart from the foregoing written agreement have been made.
